1. Scope of This Policy
Clineva Management Services, LLC ("Clineva," "we," "us," or "our") provides this public website. This Website Privacy Policy explains how information may be handled when you browse the website, use its links, or contact Clineva through the contact information displayed on it.
This policy applies to website information. It is separate from Clineva's HIPAA Notice of Privacy Practices, which describes how protected health information ("PHI") may be used and disclosed in connection with clinical care and explains your rights concerning that information.
Other websites and services linked from this website may provide their own privacy notices. Their notices apply when you use those services.
2. Information That May Be Handled
Information you provide
You may provide information when you call or email Clineva, ask about a clinic or service, or otherwise communicate with us. Depending on the interaction, this may include your name, contact information, the clinic or service involved, and the contents of your communication.
Do not send medical records, detailed health information, payment-card information, or other sensitive information through general email. Use the channel identified by the clinic for medical-record, billing, or clinical communications.
Technical information
When you visit the website, the systems used to deliver it may receive technical information such as your Internet Protocol address, browser and device type, operating system, requested page, referring page, access time, and technical error, performance, or security information.
Information from linked services
If you choose a linked booking, map, email, telephone, or other third-party service, that service may collect information directly from you under its own privacy notice. Clineva may receive information from a booking or healthcare service when reasonably necessary to coordinate or provide requested care.
Sources of website information
Clineva may receive website information:
- Directly from you or a person acting for you when you call, email, or otherwise contact us.
- Automatically from your browser or device and from the systems that deliver and secure the website.
- From clinics and service providers that support the website, communications, or a linked service, when permitted by law.
3. How Information May Be Used
Website and contact information may be used to:
- Deliver website pages and requested information.
- Respond to questions about locations, services, pricing, appointments, or records.
- Connect you with a clinic or service you request.
- Operate, maintain, troubleshoot, and secure the website.
- Detect misuse, fraud, security incidents, or technical failures.
- Maintain records required for operational, legal, regulatory, or contractual purposes.
- Comply with applicable law and establish, exercise, or defend legal rights.
4. How Information May Be Disclosed
Information may be disclosed to Clineva clinics and healthcare professionals when needed to respond to your request or coordinate care; to companies that provide hosting, security, communications, maps, technical support, or other services necessary to operate the website; or when required by law or reasonably necessary to protect rights, safety, systems, or property.
Information may also be transferred as part of a merger, acquisition, financing, restructuring, sale of assets, or similar organizational transaction, subject to applicable legal restrictions.
When information is PHI, its use and disclosure are governed by applicable health-information law and Clineva's HIPAA Notice of Privacy Practices, not solely by this Website Privacy Policy.
Mobile information and SMS consent
Clineva does not sell or disclose mobile telephone numbers, SMS opt-in records, or SMS consent to third parties or affiliates for their independent marketing or promotional use.
Communications providers may process this information only as needed to deliver and support messages you requested, or as otherwise permitted by law. Message frequency varies based on your appointments, account activity, and requested communications. Message and data rates may apply. See the Text Message Terms for opt-out and help instructions.
5. Linked Booking, Maps, and Device Services
Online booking
The Book Online links take you to a separate appointment-booking service. Review the privacy notice presented by that service before submitting personal or health information. Information provided there is handled according to that service's notice and any privacy terms applicable to the participating clinic.
Google Maps
Location pages may load an embedded Google Map or link to Google Maps for directions. Google may receive technical information and information about your interaction with the map. Google's terms and privacy policy apply to its services.
Telephone and email links
Telephone and email links may open applications on your device. Your carrier, email provider, device provider, or application provider may handle information under its own policies.
7. Retention and Security
Information is retained for periods appropriate to the purpose for which it was received and applicable operational, legal, regulatory, contractual, security, and recordkeeping requirements.
Clineva uses administrative, technical, and organizational safeguards designed to protect information under its control. No internet transmission, storage system, or security control can eliminate every risk.
8. Your Choices and Privacy Rights
You may choose not to provide information through general website contact channels. That choice may limit our ability to respond to a request.
Depending on where you live and the information involved, applicable law may give you rights to request access to, correction of, or deletion of certain personal information, or information about how it is handled. These rights may be subject to exceptions and verification requirements.
To submit a request concerning personal information covered by this Website Privacy Policy, email hello@clineva.com. We may request information reasonably necessary to verify your identity, authority, and relationship to the information before responding.
Rights concerning PHI and medical records are described separately in the HIPAA Notice of Privacy Practices.
9. Information About Children
A parent, legal guardian, or other authorized adult may contact a clinic or arrange care for a minor. Children should not independently submit personal or health information unless permitted by applicable law and the clinic's procedures.
10. Changes to This Policy
We may revise this Website Privacy Policy as website features, practices, or legal requirements change. The date at the top identifies the current version.
